Boone was born in 1981. When he was a kid, his nanny, Theresa, broke her neck on the stairs, something which Boone carried guilt for all his life.
In 1992, Boone's mother (Sabrina) married Shannon's father (Adam Rutherford). Over the years, Boone and Shannon's relationship crossed the boundaries of brother and sister. Boone had a crush on Shannon, and felt a sense of protectiveness for her. He bailed her out of trouble more than once, and although they didn't always get along, Boone took care of Shannon. They had an odd hate/love sort of relationship.
Boone traveled to Australia to rescue his step-sister from an abusive relationship. It turned out, this was only a ploy by Shannon to get money out of Boone, and Shannon had been doing it for years. In the end, Shannon was also conned by Bryan. In a moment of weakness, Boone slept with Shannon, but afterwards, she didn't want much else from him, and they ended up traveling back to the States together as brother and sister.
On the island, Boone wants to show his heroism, but doesn't succeed with Rose and Joanna; it ends up being Jack who has to save the day. Shannon snidely calls him "Captain America."
He soon becomes Locke's right hand man. After Shannon starts a relationship with Sayid, Boone is jealous, and Locke ties him and drugs him. He has a vision that Shannon is killed by the Monster, but realizes later it is not real; Locke tells him that he did it so he would be able to let go of her.
Together, they explore the island, hunt, and discover the Hatch. Unable to open it, Boone and Locke go on a mission through the jungle to find a plane Locke saw in a vision. Eventually, they find the Beechcraft plane perched on a cliff, and Boone climbs into the canopy where the plane sits to see if there is anything useful inside. Boone discovers the radio is still working, and makes a mayday call. Just as a response is coming in (not known to Boone at the time, this is just Bernard, on the other side of the island), the plane falls off the cliff, and Boone is fatally injured. He is carried back by Locke, who disappears shortly afterwards.
Jack fights frantically to save him, but finds that he can do little for him after transfusing his own blood to save him doesn't work. As a last resort, Jack considers brutally amputating his leg with a cargo door from the wreckage. Sun objects to this, believing that Boone should be allowed to die in peace; at the last minute, Boone becomes lucid and says this is also his wish. His last thoughts were of Shannon, but he never is able to articulate exactly what he wants to say.
He dies at the same time as Aaron is born. Locke believes his death was a "sacrifice" to the island.
