Little is known about Libby's past aside from the fact that she was once admitted to SRMHI (at the same time as Hurley's admission), and that she once met Desmond as a stranger, struck up a conversation with him about his race around the world, and ended up giving him a boat]boat[/url].  She said this boat was named after her ("Elizabeth") and belonged to her deceased husband David, who she says had passed away one month previously from sickness.

Libby is with the tail section survivors from Flight 815. She helps watch Emma and Zack until they are kidnapped, and sets Donald's broken leg, but later buries him after he succumbs to infection (she tells him a story about breaking her own leg skiing in Vermont at this time, to distract him). 

On the way to the Arrow, Libby talks with Michael and tells him there were 23 survivors from the tail section of the plane. As they hike to the middle section survivor's camp, Libby talks to Sawyer and tells him his wound isn't good, but that it isn't too bad either. When Sawyer passes out, she helps Michael and Jin with the stretcher.  She claimed that she had once gone through a year of med school before dropping out to pursue a career as a psychologist.

Although she originally told Hurley that they met when he stepped on her foot while boarding the plane this was most likely a lie, since he was the last one to board the plane, and she was sitting in the back already. Hurley never recognizes her from SRMHI.

She is able to help Claire recover some of her memories through relaxation techniques.

Hurley starts to trust Libby, and confides in her about his eating disorder.  When Hurley is distraught over his visions of Dave, Libby kisses Hurley and convinces him that she has feelings for him "that are real".  She prevents him from jumping off a cliff.

Libby is shot in the abdomen by Michael when she accidentally stumbles in on him killing Ana-Lucia (she was getting blankets for a picnic date with Hurley).  Jack, powerless to help her medically, injects her with heroin from the Virgin Mary statues for pain control.  Before she passes away from massive abdominal injuries, she utters the last words "Michael."  Jack and others misinterpret this as concern over Michael's safety, not that she is trying to tell them her killer's name.
