Aaron is the child of Claire and Thomas, though Thomas left Claire soon after she became pregnant. Distraught, Claire decided to place Aaron for adoption. Malkin, the psychic, warned her against leaving the baby, telling her that the baby needed to stay with its mother. However, when Claire made it known she wasn't keeping the baby, the psychic soon changed his story and convinced Claire that there was a family in LA that wanted to adopt her child. The psychic paid her to travel to LA aboard Flight 815.
On the island, the baby doesn't kick during the first few days after the traumatic crash, and Claire is concerned; but after eating some sea urchin caught by Jin, Claire is ecstatic that he starts to kick again.
During Clare's pregnancy, she is kidnapped briefly by Ethan and the Others. At this time, Ethan injects the baby with some kind of medication, which he calls a "vaccine," through Claire's abdomen. The Others have plans to take Aaron by c-section, but are thwarted when Alex helps Claire to escape; the rest of her pregnancy is uneventful.
Aaron is born about the same time Boone died. Since then, Claire has reversed her position, and now desperately wants to keep her baby. Both Charlie and Locke become something like father figures, helping Claire take care of Aaron. Sawyer's voice has a calming effect on the baby.
When just a few days old, Aaron is taken by Danielle in an attempt to exchange the baby with the Others for her own kidnapped daughter, Alex. This attempt fails when Danielle is unable to find the Others and is caught by Sayid and Charlie, who return Aaron to her mother.
When little Aaron several weeks later gets a rash, Claire becomes convinced that this is of concern, though Jack believes it is just a common childhood illness that will go away on its own. Claire is still worried, and ends up going with Danielle and Kate to the Staff Station to find the medication she believes her baby may need. Danielle tells her that she hopes her baby is ok, but that if it does turn out to be the sickness, then she hopes Claire will know "what she must do." Aaron's rash appears to have gone away, but Charlie has found an injector with medication in the food drop, which he gives to Claire. Desmond tells her that she is wasting her time with it, because he shot himself with it every 9 days for 3 years.
