Juliet was once a medical researcher who worked for her ex-husband, Edmund Burke's laboratory in Miami. She was secretly stealing experimental medications to help with her sister Rachel s illness, though she was once caught in the act by Edmund and his new lover, Sherry. She was known for her obstetric research in infertility, with one of her credits being that she managed to get a male mouse impregnated. However, when Richard Alpert came to recruit her for Mittelos Bioscience, she turned down the job, because she cited she was "not a leader". She unseriously said she would only be able to work there if her ex-husband got hit by a bus. When this unexpectedly happens for real, she is shocked. When Richard Alpert and Ethan Rom (who was also working for that lab) try to recruit her a second time, she is more open to the idea of joining them.
Juliet seems to be pretty high up in the hierarchy of The Others. Prior to Flight 815, she ran their book club. It also appears she has a strained relationship with the leader of The Others, Ben. Despite this, they manage to work together to accomplish their goals (such as finally breaking Jack).
However, once Jack, Kate and Sawyer are captured, we see that Juliet takes a more prominent role. She is apparently assigned to break Jack, which she does by diving into his personal life. She appears familiar with medications, and has a personal file on him. She also responds to the escape warning when Karl and Sawyer break out of their cages. She incapacitates Sawyer and sees that he is returned to his prison.
On the surface, Juliet is working with Ben, but she secretly asks Jack to kill him, saying on a silent secret video that he is a dangerous man, and Jack should perform a spinal surgery on him, but botch it on purpose. Jack ends up performing the surgery and does nick a kidney vessel on purpose in order to buy his friends enough time to escape. However, he also fixes this and stitches him up afterwards, and reveals Juliet's betrayel to the Others. Juliet went to find Sawyer, Kate, Karl and Alex, just as they are about to escape in Alex's boat. She saves them by shooting Ben, and then convinces Alex to stay by saying that Ben might be upset if he wakes up without her there; Alex ends up staying to save the other three.
