He didn't have a little cut on his face, but a huge gash.  I guess being on the island without proper medications and bandages to help it heal perfectly would most definitely leave a scar like that.

I have allways been interested in the scar on Johns eye. I dont know if this is a "stab in the dark", but I have seen this type of scar depicted excessively in Japanese pop culture (Manga, video games, anime? ect...). I am not really versed on this subject but I can assume that it holds some sybology in their culture, I just dont know the refrence. Or, I could be mistaken.

It portrays something about the character. Either badassness, evil, capacity for evil, toughness... its all here:
