I believe that Sarah (Jack's ex-wife) was an off-Island Other whom had come to civilzation to "spawn" (for want of a better word).
Juliet knew her. Juliet is a fertility expert.
When did we find out Juliet and Sarah knew each other? I don't recall that at all... Well, I'm not saying she knew her as a friend, but rather she knew of her... http://lostpedia.wikia.com/wiki/S3E01_-_A_Tale_of_Two_Cities_TranscriptJULIET: Jack. Jack. What would you like to find out?
JACK: Is she -- is she happy?
JULIET: Yes, Jack, she's very happy. Yeah....that's...totally different. She was reading from a file she was given about Jack's life. Kind of a stretch to assume that means Sarah was an Other....

I wonder if drilling into that rock helped make pregnancy not work on the island. It would explain how "Marvin Candles" kid was born there (assumed) and Penny likely as well.
It does make me wonder about the mother. Maybe its Eloise? who knows yet.

I know she had a file on Jack, but I doubt the file would use adjectives such as "happy". You know, like: "Subject purchased white grapes, iceberg lettuce and french dressing at 17:40 hrs from local convenience store. She looked blissfully happy as she did so."If you see what I mean. To know if someone is or isn't happy, you have to know them.

You know, though, even if she is dead, we'll still see more of her. This show has a tendency to ignore standard rules and timelines

To know if someone is or isn't happy, you have to know them. There is absolutely no reason to think Juliet and Sarah know each other in any way shape of form. You're looking for stuff that isn't there. If the file says she's in a stable marriage, is gainfully employed, etc. and Jack asked if she was happy, the answer would be "Yes." Because it makes for better TV than having Juliet say, "Well, one would think so based on the limited information we have in her file..."

Maybe babies were able to be born on the island before "the incident" talked about in the swan video. We've never been privy to what that incident was, were we?

So if she was probably (or possibly) born on The Island... did she grow up there? When did she leave? Did she leave when she was so young that she couldn't really remember it (like Charlotte or Miles)? At minimum, finding out that Charles was on the Island for at least 30 years tells me that Penny knows more than we ever imagined.

Episode S05E08 revealed that pregnant DHARMA women went off-Island to give birth (if not actually procreate). http://lostpedia.wikia.com/wiki/LaFleur_transcriptDOCTOR: I'm an internist, not an obstetrician. Our women always deliver on the mainland. Amy was supposed to get on the sub on Tuesday. The baby's two weeks early.
[Amy screams a terrible scream.]
SAWYER: Can you deliver this baby or not?
DOCTOR: I don't know.
[Sawyer rushes out the door.]

[Sawyer rushes out the door.]ok, I haven't seen the show yet; had to work late, and it was unexpected so I didn't record it but if that's what happened, then that's just dumb he was going to put a pregnant woman on sub 2-3 weeks before she's scheduled to deliver? even though he's "just" an internist, he must have heard about premature deliveries in med school what would they do on the sub if she had gone into labor there? it's got to be less risky to let the internist deliver the baby than to put her on a sub 3 weeks prior to her due date
