I thought the whole thing was leading to Jack trusts Kate completely and didn't trust Juliet completely.  However, that he pretended to be asleep makes me think he trusts no one.

Yeah. It was like Juliet's way of giving the couple her blessing and letting Jack go.

Juliet is totally my favourite character on the show. I find her more intriguing than Ben to be honest.

The background music when she spoke was sort of ambiguous though. It was kind of sinister (as if to imply that she was trying to stir up trouble) but really she was doing the decent thing.

In fact, sinister music seems to underpin most of Juliet's scenes, when in fact she's the only Other whom isn't remotely sinister! She's so misunderstood!

I love that Juliet has her own agenda and nearly always remains calm under pressure.

So, what was up with Jack INSISTING that Kate be in the room during the operation, and wanting to be awake.  He would appear to understand Juliet better than most, and apparently does not trust her (don't blame him).  For all of Kate's faults, he does know he can trust her to have his best interests at heart and to care about him (she did put herself at risk already once to save him).
Isn't it possible that he was afraid that Juliet would do something sneaky, like planting something in his gut, and that if Kate were there she would not have allowed it?  Very possible that Juliet is still working for Ben... That would give Ben (or the island) "control" over Jack once he leaves the island...

Bernard was still in the operating room the whole time, so obviously no device-planting happened.

I'm pretty sure Jack trust Juliet. They just kissed a couple epis earlier.

What was Bernie doing? Handing her stuff. When would Juliet have the time to sterilize something and place it inside Jack while Bernie's back was turned? I've never been in an operating room, but I'm assuming they are generally rather delicate procedures.

I just felt that Juliet was acting a little weird and I still don't trust her even now.  I love Jack but he is so anally retentive sometimes.  I guess Bernard knows all about injections cos would he have had to give Rose stuff at some point when she wasn't well?

Not sure about that but thought the effects of how they showed the cut and the sutures was really authentic.
