As to the first question, I guess they could have used more dramatic flashes (as they did in his flashbacks from "Cost of Living")... but well, that didn't bother me. Dreams are like that, you never know what parts your subconscious mind could jump to.
For the second question, Eko dropped his cross in the dirt when he was young (and taken by the militants). His brother later picked it up. That's the scene it was from, not the later nonflashback scene where Eko takes it from his brother.
